Graduate Students: All Graduate Division Forms must be signed by your major field advisor and research advisor in the margin prior to acquiring the authorized signature from ONLY the "Head Graduate Advisor" aka Vice-Chair of Graduate Study. The Graduate Student Affairs Officer will obtain the signature of the Vice-Chair for you and forward the completed form to the Graduate Division. The Graduate Division will return all forms with unauthorized signatures.
